Our Criteria
Eligibility

Applicants must be at least 18 years of age.

Enrollment in Medical Assistance (MA) is required.

A documented disability or long-term condition must be present.

Support needs must result from a disability or indefinite condition.

Individuals should face housing instability or distress.

Must be at risk of becoming homeless.
